Wrong! This is what it should be:

begin;
SRL ID:'';//Your Srl Stats Id
SRL PASS:'';//Your Srl Stats Password
end.

Your form name and pass are completely separate... Read my How To Get Srl Stats Tut (link is in my sig) if you don't understand.
