Normally if you need help with a script, its best to ask for it ON THE THREAD WHERE YOU DOWNLOADED IT FROM.
instead of
MAKING NEW THREADS WHICH TAKE UP ROOM

Yea its a VERY old script. Also when you need help on a script post on the thred where the script is released. Lastly they are called player forms. You just use cross-hair then click play and a box for your username & pass and other junk will pop up.
