Hi, I need someones help to
1. Print off a form
2. Fill it in
3. Scan it, and email it to me

I dont have a scanner so i cant do this :(

I am more than willing to give you a mill or so for your time.

lol, i forged his signature!
actually i didn't. I got a co-worker to forge it since i suck at cursive. My co-working wouldn't forge it ON the form though. So he just wrote it on a paper and i went to a window, held the two pages up, and copied it. HAHa (:
