No SRL stats account entered, would you like to set one up

Wtf is this? When I try to run a script this comes up. It tells me to reload the script and fill in the form, but there is no form? I'm trying to run the MSI miner.

You just click on "No" and still use the script. Personally, I'd like you to set one up though :)

Umm, that's not really the point. I setup like 2-3 of them, but I don't know where to enter it, because there's no form :|

And are you sure I can even use the script? Because when I press no it just sits there doing nothing..

How is that not the point? I explained what that was, and how to work around it. When you run MSI, there are boxes on the bottom right of the very first screen that appears where you put your SRL stats credentials.

Oh, now I see the form thing, but the program just sits there, doesn't log in, doesn't do anything? Do I manually login or..?

Make sure you are filling everything in correctly. Look over the form again. Also, the login servers are messed up right now so you have to wait for Jagex to fix them.
