I have recently downloaded smart manager and running two different scripts over night. Upon waking up in the morning I come to find my firefox browser open on the RS website and signed into one of my accounts, looking at the recovery questions as if it was trying to change them.

The first night this had happened I was running an account that had been made that day. The recovery questions page was up and when I tried to login, runescape found that it had been locked.

It happened again last night with a different account. Whats going on here?

This is a bug, it happens to me every once in a while. It didn't do anything to your account, it just kept clicking on the Recovery Question section of the lobby. Has to do with the LoginPlayer procedure but doesn't happen too often.

This happened to me too. It happens on accident. When the script fails or if you lag and log out. It attempts to log you back in but will accidently click on "recover account" or "forgot pass" link (forgot what its called)

Make sure you have a fully updated SRL.
