Default Need help with user input

There used to be a power miner that I used but don't have anymore that would come up with a little text box asking for user input like username and password. What commands would I use to make this come up. I was thinking on getting information that would change often so it would be more effective to just enter in the info every time. Any help with the commands that make this happen would be great.

Look up on a tutorial on "forms" (without the quotes) and "TEdit"s (without the quotes). That is what they're called
