I agree, there are so many people that deserve to be on such a list that I don't even want to make one.
I would only include people whose code (or at least of it) I have seen and therefore could judge, though. (SMART, and such don't count, imo... Sorry ben. <3

everyone forgets Nielsie95. He just happens to be magical, thus not needing skills since he just needs to will it so and his program will write itself.

(r0b0t1) and (Yakman + Yakwoman) also are probably up there.

I would assume freddy because of SCAR, etc, and Wizzup because of various web programming things.

You did however specify general programming knowledge. While someone may be super knowledgable of programming and know things, they may not be able to apply their knowledge to creating a program. On the other spectrum, someone may know very little, but appear to be knowledgeable through his crude but effective programs or heavy use of outside help without attaining any knowledge themselves. This isnt a clearly cut question with a definite answer, it is a very general and hugely biased poll.
