It's not hard to know where items are in your inventory...or look at Uptext when you scroll over something.
They can't detect it. You'll be fine botting.
Im using idungeon pro too. It works fine with the items deformed and that have disappeared. Im very impressed that it does.
When you bot you are taking a risk, If you do not feel comfortable with what it is doing just don't use it :P

I was thinking this, but I haven't actually heard about any bans or rollbacks. I guess we take the risk everyday.

As said above, technically selecting the right item in your first attempt even though its invisible doesn't favour injection bots at all...

Its cause you are on Safe Mode, switch to something else and the items will be normal again.
