They change the game code and change item ids.
There can also be subtle changed to the map and colors.

If you are talking about major bot nukes there is so far only 1 bot nuke (last year Oct) which they simply obfuscate the code (breaking reflection&injection bots temporary), other than that it would be minor changes to break bots, like dynamic colors etc. As they said, the battle against bots is a continuous challenge (i.e. they can never succeed)
Exactly what riwu said. To add on to this, Optimus was a platform implanted into Runescape back in May. It should act as a tool for Jagex to deal with bots. Maybe this implies updating the client/hooks and such while the player is still logged in (without the need to restart the client) or simple a more advanced macro detection program. Who knows...

Whatever they actually did with Optimus, it's not working well.

They say Optimus will help them update better and as we all know, updates break bots for a bit.
